2 Days

5 Core Modules

Gain the knowledge and skills to facilitate RCA and incident investigations utilising Apollo RCA™ methodology and Reality Charting® / RC Pro® software - from defining the problem and analysing cause and effect, through to identifying effective solutions and implementing and tracking outcomes.

Introduction to Problem Solving and the Apollo RCA™ Methodology

  • Understanding ineffective problem solving
  • Differentiating between Issue Resolution and Issue Elimination and Prevention
  • Overview of the Apollo RCA™ four-step methodology
  • Understanding Cause and Effect Principles
  • Introduction to RealityCharting® / RC Pro® software (each attendee will receive their own RC software licence with access for 12 months)

Step 1 - Define the Problem

  • Understanding the elements of Problem Definition
  • Understanding the Importance of the "Significance" and RCA Threshold Criteria
  • Exercise: Apply Problem Definition to real-world case studies

Step 2 - Cause & Effect Analysis and the Reality Chart™

  • Understanding the Principles of Cause & Effect
  • Learn how to Identify and Arrange Causes
  • Learn how to create a RealityChart®
  • Understanding the Importance of Evidence
  • Exercises: Apply Cause & Effect Principles and Analysis to real-world incidents

Step 3 - Identify Effective Solutions

  • Know the Criteria of Effective Solutions
  • Apply Creative Solutions Process and determine Root Causes
  • Understand importance of acceptable risk relative to problem solving
  • Exercises: Apply Cause and Effect Principles and Analysis to complex case studies

Step 4 - Implement, Track and Facilitate

  • Know the elements of an Effective Report
  • Know how to Implement and Track Solution Effectiveness
  • Be Able to Facilitate an Incident Investigation
  • Develop group facilitation skills to lead teams through high-complexity RCA investigations
  • Understand Common Traps
  • Capstone two-hour team case study exercise (Employee Injury or Plant Shutdown)

Learnings are reinforced with instructor facilitated “hands-on” exercises that allow students to apply the Apollo RCA™ process to real world problems, and in doing so demonstrate their facilitation skills.
